Be part of a dynamic health care setting serving frail elders in Westerly, Rhode Island Day Center RN - Full time-Under the supervision of the Nursing Supervisor, the Day center Nurse's responsibilities include coordinating care, collaborating with the clinical team, physical assessments, providing treatments, emergent care and documentation for PACE and Westerly Adult Day clients. In addition the Day Center RN, supervises the Day Center Med Tech and C.N.A's. Communicates with participants and families regarding health status and emergent visits. Completion of an Associate's Degree in Nursing (ADN); Bachelor's degree preferred. Cur rent RI state licensure as a Registered Nurse. Minimum one year's experience as a Registered Nurse. Homecare C.N.A- Part time - provides personal care and services according to individual care plans for participants of the PACE Organization of Rhode Island. A high school diploma or equivalent is required. Current C.N.A. certification required. Minimum one-year experience working with frail or elderly population. Valid and current driver's license. Current CPR accredited by American Heart Association and American Red Cross. Reliable transportation.Valid and current automobile liability insurance.
