Service Coordinator

Our Adult Day Health Center located in Taunton is seeking a full-time Service Coordinator directly responsible for the coordination and provision of clinical social work services within the program. Works in conjunction and collaboration with the multi-disciplinary team. Participates in community outreach and networking with area agencies. Demonstrates knowledge and implementation of regulatory requirements and company policies and procedures.
Qualifications: Degree in Social Work licensed to practice in teh Commonwealth of Massachusetts; at least two years related experience, inclusing counseling, family and elder services, or relevant experience; CPR Certified and First Aid training required.
