Youth Outreach Program

Our History 
Tides was founded in 1983 by Brother Michael Reis, a DeLaSalle Christian Brother,  with $15,000 seed money.  The Lasallian mission is to serve the neediest and poorest families in the community.  Brother Michael realized interventions needed to happen before kids were placed in out-of –home care.  He founded a much-needed “agency without walls” where outreach workers and clinical social workers meet with and provide critical services to youth in their homes, neighborhoods and schools.  Today, we provide 24/7 services 365 days a year through 120 staff and an $8 million budget.


Tides’ Youth Outreach Program  provides individual and family counseling and crisis intervention services for youth, both male and female ages 9-17 and their families in West Warwick, Warwick, Coventry, Providence and Blackstone Valley. The purpose of this program is to keep at-risk youth from becoming involved with the Rhode Island Family Court and the Department of Children, Youth and Families. Referrals are made by parents, Family Court Diversion workers, Juvenile Officers and school departments.
 

  • Providing community/home based assessment and informal counseling (individual, group and family) to assigned clients. 
  • Maintain a weekly census of a minimum of 70 cases between both sites for the Youth Outreach Program;
  • Provide case management according to individual treatment plans to assigned clients including, but not limited to: referrals to ancillary services, participation in the development and implementation of mechanisms to address treatment barriers such as transportation and adaptive daily living skills, and participation in the development and implementation of mechanisms to facilitate client’s integration into the community;
  • Provide crisis intervention services by assessing individual and/or family crisis, provide information and link short-term interventions within the available continuum of services;
  • Participate in the development and evaluation (Utilization Review) of individual treatment plans;
  • Individualized treatment plans and level of service delivery will be determined by program Supervisor/Director;
  • Maintain accurate client files including but not limited to, appropriate releases of information, permission forms, informed consent forms, treatment plans, treatment notes, utilization review forms, and treatment and closing summaries;
  • Coordinate with internal and external services to maintain open communication and a best practice continuum of care;
  • Provide community/home based informal counseling, according to individual treatment plans, to assigned clients Any additional duties assigned by the YOP supervisor. 

Requirements:
BA or BS
Valid driver’s license and registered/inspected vehicle.
