Day / Vocational / Community Based Service Coordinator
Looking Upwards is a Middletown based non-profit organization supporting individuals of all abilities. Looking Upwards Day/Vocational and Community Based Family Services is seeking to hire a Service Coordinator. Our day/vocational and community based family services is a group of people working toward a vision of the world in which all people have positive control over the lives they have chosen for themselves. Through this shared commitment and the Individual Service Planning process, we focus on choices, community, employment, health, safety, learning, friendships and personal growth. The service coordinator works directly with the person, their family and the team to create a individualized plan supporting the person to achieve their life goals. The following qualifications are necessary:
- Have a degree in voc rehab, psychology, social work or related human service area
- Experience working with individuals with disabilities preferred
- Must have proven skills in managing a caseload with a focus on ensuring that each person’s goals are met
- Must demonstrate excellent family centered practices
- Past leadership/supervisory experience preferred
- Must have strong organization skills and have the ability to multi task
- Is solution oriented with strong communication and interpersonal skills
- Experience and proficiency in designing, implementing and overseeing plans
- Critical thinking skills, creativity and innovation a must
- Must have excellent writing, computer and editing skills
- Must have ability to interface with multiple teams, family members, Division Social Workers
- Must be service coordinator on call for voc/community based family services weekends on a bi-monthly basis and as needed
- Is energetic, can work autonomously and is self-directed
- Ability to effectively facilitate meetings with staff and families
- Ability to be an effective trainer
- Must be flexible and available to work the varied hours to meet the needs of the program including weekends and evenings
- Must demonstrate the ability to prioritize needs of programs
- Ability to work in a fast paced environment
Additional requirement for employment eligibility include:
Must have a valid driver's license with at least 3 years driving experience and willing to use personal vehicle
