Case Manager

General Description:

The mission of YWCA Rhode Island is to eliminate racism, empower women and promote peace, justice, freedom and dignity for all. The Case Manager supports the mission of YWCA by performing a variety of casework duties for residents at the Gateway to Independence I & II transitional housing facility.

Responsibilities:

The Case Manager is responsible for developing a service plan for each resident and assisting the resident in achieving his/her goals.   The Case Manager reports directly to the Chief Operating Officer.

Essential Duties:

  • Screens potential new residents
  • Arranges meeting with assigned resident within 24 hours of admission/intake
  • Creates a face sheet and case file for each assigned resident
  • Develops a service plan with defined tasks for each assigned resident
  • Maintains and updates confidential record keeping functions and administrative duties required by the program (i.e. case file, case record, face sheet, file cards, daily census, HMIS, etc.)
  • Performs a variety of casework duties including obtaining, analyzing and evaluating data related to cases
  • Creates regular reports on the progress of each resident including noticeable changes in appearance and behavior
  • Identifies resident needs for external services and contacts agencies regarding the resident’s situation and helps resident establish a relationship with service provider
  • Explores the resident’s eligibility for benefits and assists in obtaining benefits
  • Cooperate with other service providers to obtain housing, health and social services for resident
  • Assists resident in arranging necessary appointments
  • Assists resident in enrolling in training programs, education programs, finding employment and/or volunteer service
  • Establishes and monitors a case tracking system for each resident
  • Follows, maintains and implements all house policies and procedures
  • Special projects and events as assigned
  • Drive program vehicles when needed
  • Represent organization in community initiatives and support the mission of YWCA

Non-Essential Duties:

  • Attend scheduled meetings, staff supervision appointments and special events as assigned
  • Participate in recommended training programs
  • Perform miscellaneous job related duties as assigned by Supervisor, COO or CEO

Minimum Qualifications:

  • Bachelor’s degree from an accredited university in an appropriate discipline such as: social work, psychology, or human services or another related field
  • Two years experience in human services or a related field
  • Knowledge of veteran specific issues preferred
  • CPR and First Aid Certified, recommended
  • Respect and consideration for the residents being served
  • Excellent administrative and organizational skills
  • Strong verbal and written communication skills
  • Ability to maintain highly confidential information and deal with sensitive issues in a responsible manner
